Female Pattern Hair Thinning
24 weeks of the one over-the-counter treatment proven in trials for female pattern thinning, with Afro-hair-friendly application and a doctor route.

- Length
- 24 weeks
- For
- Women
- Evidence A
- Strong evidence: guidelines or reviews of trials
- What to expect
- Extra shedding is possible in weeks 2–8 (hairs re-entering growth). First visible change: 3–4 months. Judge results at 6 months. Stopping loses the gain within months.
The goal: For most women shedding slows and thinning stops getting worse; more women see regrowth than with placebo (about 26% vs 14% in trials). The benefit lasts only while you continue.
Topical minoxidil is backed by a Cochrane review and the EDF S3 guideline (Grade A). Trials were almost all in white women; Afro-hair application and styling advice is expert consensus (Grade C).

The plan
Weeks 1–2
Confirm & prepare
Rule out other causes, take baseline photos, patch test.
Thinning at the crown or parting can also be CCCA (scarring), traction or shedding, which need different care.
Morning
Monthly hair photos
Same window light, hair dry and loose: middle parting, crown, front hairline. Same day each month.
Why: Hair changes slowly; photos are how you and your doctor judge it.
Evening
Gentle wash & condition
Wash every 1–3 weeks (more often if a daily scalp treatment makes the scalp oily). Condition every wash. Detangle wet, with fingers or a wide-tooth comb, from the ends upwards. Fingertips on the scalp, never nails.
Why: Coily hair breaks most when dry-combed; BAD advises washing Afro-textured hair every 1–3 weeks.
BAD patient information leaflet: Caring for Afro-textured hair (July 2023)
Habits
Doctor check first if…Doctor or pharmacist
Crown thinning with itch, burning or tenderness; smooth shiny patches; sudden heavy shedding; irregular periods, acne or new facial hair (possible hormone cause); or you are pregnant, trying, or breastfeeding. A skin doctor can examine the scalp and arrange blood tests.
Why: CCCA, hormone problems and shedding look similar but need different treatment; the scalp treatment in this program is not for pregnancy or breastfeeding.
Patch test the minoxidil product
Before week 3, put a small amount on the scalp behind one ear once a day for 2 days; check for itch, redness or rash. Not in pregnancy or breastfeeding: stop and tell your doctor if you become pregnant. Not for under-18s.
Why: Some people react to the solution base (propylene glycol); foam is a gentler option.
Buy genuine, from a registered pharmacy
Check the NAFDAC number and expiry date. Avoid unlabelled "growth oils" or mixtures sold as hair-loss medicine — they are often not what they claim.
Why: Fake and diluted products are common; the real effect comes only from the tested medicine.
Food
Protein & iron foods
Beans, eggs, fish, meat, groundnuts and leafy greens (ugu, efo) every day. No crash diets.
Why: Low protein and iron deficiency worsen shedding; a doctor can test ferritin.
Weeks 3–12
Treat consistently
Daily minoxidil on the scalp; gentle Afro-hair care.
Minoxidil lengthens the growth phase of hair. Trials show benefit over placebo; 2% and 5% work about equally in women.
Morning
Minoxidil 2% solution twice daily OR 5% foam once daily
Scalp must be dry. Part hair into rows 1–2 cm apart. Use the dropper (1 ml total for the solution) or half a capful of foam, applied to the SCALP of the thinning area only, not the hair. Spread with fingertips, then wash your hands. Leave at least 4 hours before washing or swimming. With braids or cornrows, apply along the partings. Foam (no propylene glycol) is usually less itchy and less greasy on Afro hair. Not in pregnancy or breastfeeding: stop and tell your doctor if you become pregnant. Not for under-18s.
Leave on: scalp dry; 4 h before washing
Why: Cochrane: minoxidil improves regrowth and hair counts compared with placebo, and 5% is not better than 2% in women. The EDF S3 guideline recommends 2% solution twice daily or 5% foam once daily.
Evening
Minoxidil 2% solution — second dose (solution users only)
If you use the 2% solution: second 1 ml dose to the dry scalp of the thinning area, at least 2 hours before bed so it dries and does not rub onto your pillow and face. Foam users skip this (foam is once daily). Wash hands after. Not in pregnancy or breastfeeding: stop and tell your doctor if you become pregnant. Not for under-18s.
Leave on: dry before bed
Why: The 2% solution regimen used in the guideline is twice daily; drying before bed reduces unwanted face hair.
Protect for sleep
Loose plaits, twists or a pineapple; satin bonnet or satin pillowcase. Wash the bonnet weekly.
Why: Less friction breakage, which makes thinning look worse.

Habits
Expect and ride out early shedding
Weeks 2–8 of treatment may bring extra shedding. Keep going unless you have a stop sign below.
Why: Resting hairs are pushed out as new growth starts; stopping early is the commonest reason the treatment seems to fail.
Unwanted face hair?
Fine extra hair on the forehead or cheeks can appear, more with 5%. Apply carefully, wash hands, let the product dry before lying down, keep it off pillows. Tell your pharmacist or doctor; it usually settles if you stop.
Why: A known, reversible side effect in trials.
Styles while treating
Choose loose styles. Braids out within 6 weeks, weaves or extensions within 8. Leave edges out; no glue on the hairline. Pain after braiding means too tight — you should never need painkillers.
Why: Traction on top of thinning adds hair loss; these limits come from BAD Afro-hair guidance.

Weeks 13–24
Keep going & judge
Continue; compare photos at 3 and 6 months; doctor review if no change.
Results are judged at 6 months; gains are kept only while treatment continues.

Doctor review: other optionsDoctor or pharmacist
If the scalp treatment is not working or not tolerated by 6 months, a doctor may discuss tablet options such as low-dose oral minoxidil, after checking your blood pressure and heart history. Your doctor will advise the dose and duration. Not in pregnancy or breastfeeding.
Why: A 2025 international expert consensus guides when and how doctors start low-dose oral minoxidil; it needs medical screening.
About hair supplements
Biotin and "hair vitamin" gummies have no proven benefit unless you are deficient, and biotin can distort blood tests (thyroid, heart) — stop it 3 days before any test. Iron tablets only if a blood test shows you are low.
Why: Reviews find no hair benefit from biotin in people with normal levels; too much iron is harmful.
Food
Protein & iron foods
Beans, eggs, fish, meat, groundnuts and leafy greens (ugu, efo) every day. No crash diets.
Why: Low protein and iron deficiency worsen shedding; a doctor can test ferritin.
